As you be informed, from my answer to an earlier question from you, I was at school at a time when corporal punishment was commonplace. It is difficult to go into detail on a public channel like this, but as you are on my contact record, feel free to get in touch.
To answer your specific questions:-
1. Over the arm of an armchair.
2. Strap or cane. Always on the bare bottom.
3. As a rule on my own, except when four of us were caught out of bounds and we were made to watch as each was punished in turn.
4. Yes, but only for the cane or strap on the hand.

What was the most usual position for receiving corporal punishment at school?

Bending over, heart-breaking toes
Bending over, holding ankles
Bending over a chair
Bending over a desk
Over the teacher's knee

When I went to school there was no corporal punishment, but before it was bending over the desk and getting the cane on your bottom. Contented I didn't have to experience that!
